- All forms are in PDF format. Web browsers like Google Chrome, Firefox, and Safari all have integrated PDF readers. Alternatively, Adobe Acrobat PDF reader can be downloaded for free from the Adobe web site.
- Some forms are available in an editable version allowing the blanks to be filled in on the computer. The regular version must be printed and filled in by hand. The original of either must be mailed to the office.
- Clicking the “Open” link will show the form. You can then print it or save it to your computer (right click and select “Save as…”)
- If you have trouble opening or printing these forms, contact the Registrar who will be glad to mail them to you.
